Vaccine hesitancy—an impending barrier to India’s COVID-19 vaccination program

Background

Vaccine hesitancy can jeopardize India’s plan to defeat COVID-19 as it scrambles to vaccinate its population in the wake of the second wave of the disease. While the second wave seems to be receding, the possibility of an intense third wave looms large. The fear of a third wave is real because the rate of vaccination has been slow, the possibility of more virulent strains of the virus emerging is high, and the need to reopen economic activity is critical—though it may lead to people adopting COVID-inappropriate behavior, as seen from our studies in the past.

In this situation, the best possible solution to mitigate the risk of a third wave is to quickly vaccinate as many people as possible to break the chain of transmission and reduce fatalities. Enforcing COVID-appropriate behavior and building the capacity of the health system has to continue in parallel.

Currently, progress in vaccination has been slow because of an imbalance in supply and demand. However, as per the plans released by the Government of India, vaccine supply should normalize by July-Aug 2021. The plan is to have more than 2 billion vaccine doses by Dec, 2021, which is enough to vaccinate the eligible population. The availability of vaccines has to be complemented with a massive ramping up of the supply chain, vaccination centers, and healthcare staff to distribute and administer the vaccine. Even if availability, distribution, and administration of the vaccines are sorted, achieving complete vaccination of the eligible population by Dec, 2021 is a daunting task. And the reluctance of people to get a shot in their arms, or vaccine hesitancy, remains a key factor that can jeopardize the plan.

According to the World Health Organisation, vaccination hesitancy refers to the delay in acceptance or refusal of vaccines despite the availability of vaccine services. It is influenced by factors, such as complacency, convenience, and confidence. Issues of vaccine hesitancy have already started cropping up in different parts of the country and across the world. These need to be tackled on a priority basis as misinformation and complacency can spread quickly.

For example, in the USA, the seven-day trailing average of vaccination has dropped from the peak of 3.7 million a day in mid-April to less than a million per day toward the beginning of June, 2021. About 41% of the population is fully vaccinated and 51% have received at least one dose—a large part of the population remains unvaccinated. This is primarily due to vaccine hesitancy and it is almost certain that India will face this issue as the availability increases.

 

A three-pronged approach that combines interventions from both demand and supply sides is needed to manage vaccine hesitancy. Information, Education and Communication (IEC) is the most important “prong” in the approach to reduce vaccine hesitancy. It can be even more effective if coupled with the other prongs of incentives and an efficient supply chain. The following table provides a list of ideas across these three elements to manage vaccine hesitancy.

Information, Education and Communication (IEC)

IEC can be through below-the-line (BTL) or above-the-line (ATL) channels. The key is to ensure that all IEC campaigns are carefully planned, executed and monitored with active feedback loops to ensure that messages are adjusted and misinformation is addressed rapidly. MSC’s work on the effective communication of government to people (G2P) benefit payments provides important insights for this.

Below-the-line IEC channels 

  • Frontline workers: People-to-people awareness campaigns can be launched that utilize frontline workers like ASHAs, Anganwadi workers (AWWs), and auxiliary nursing midwives (ANMs). The frontline workers can either reach the eligible candidates or specific target groups directly to motivate and track them. Mechanisms for supportive supervision and monitoring may be set up.
  • Last-mile service delivery points: Campaigns can also be launched through the vast networks of citizen-facing outlets developed to deliver products and services to the rural population, such as Citizen Services Centres (CSCs), Public Distribution System (PDS) dealers, fertilizer dealers, and self-help groups, among others. MGNREGA work sites can also be used for this purpose.
  • Through reminders and telephone calls: Health personnel like Community Health Officers (CHOs), Block Community Process Managers (BCPMs), and ANMs may be deployed to speak to hesitant clients through phone calls to motivate and convince them to get vaccinated.
  • Through community influencers: Community influences like religious and community leaders, gram pradhans, panch, and ward members, among others, may be identified and engaged with. They can play a critical role in influencing the vaccine-hesitant group.

Above-the-line IEC channels

  • Regular activities that explain the importance of vaccination, its safety, efficacy, and possible side effects need to be initiated and increased. Such activities, including social media campaigns, mass media campaigns, informative caller tones particularly voiced or fronted by a famous non-controversial celebrity like sports personality or film stars, wall paintings, and hoardings could prove vital in dealing with misinformation and influencing the decisions of people to get vaccinated. A lot of action is already happening through ATL channels but it needs to increase as we vaccinate early adopters and start to target hesitant population.

Incentives

Incentives can play a key role in motivating people to take vaccines. These may be in the form of financial incentives or non-financial incentives.

  • Financial incentives: One form of financial incentive can be DBT cash transfers to the households if all the eligible members get vaccinated. It can generate healthy competition among the residents of an area or community to take up vaccination.

A lottery system could also be tried where every vaccinated person is eligible for a lucky draw once that area, block, or district reaches a certain threshold of vaccination. A few big prizes can be announced to motivate people to participate in it. FLWs could also be incentivized to get their eligible catchment population vaccinated. Gram panchayats could also be incentivized to motivate people in their jurisdiction to get vaccinated.

  • Non-financial incentives- Food packets or other goods can be provided if all members of a house are vaccinated. This may be a top-up to the already sanctioned quantity of ration through PDS. Another way could be by recognizing those households as “Adarsh” (ideal) houses, where all the eligible members are vaccinated. These households can be felicitated by the local administration with a certificate, sticker, badge, or similar token.

Mandatory vaccination status for non-essential events that attract attention can also be tried as positive reinforcement for vaccination. Such events include entering famous places of worship, unrestricted air/train/road travel, entry in sport stadiums, gyms, swimming pools, conducting functions like weddings, and holding political gatherings only when organizers are vaccinated, among others.

Supply chain

The availability of vaccine doses and its easy accessibility play a critical role in influencing the issues related to complacency and convenience, and in turn affect vaccine hesitancy.

Ensure sufficient doses of vaccines

As per the National Health Authority, the demand-supply gap in COVID-19 vaccines stood at 6.5:1 at the time of writing, falling from an earlier gap of 11:1. While a lot of work needs to be done to bridge this gap, a strong pipeline of potential vaccine candidates and enhanced vaccine production in the country suggest the gap is set to reduce further.

Access near the community

The government may utilize the existing Universal Immunization Programme (UIP), where ANMs conduct regular vaccination sessions at pre-designated places in the community, such as health sub-centers, Anganwadi centers, and Panchayat Bhawans. In addition, the newly developed health and wellness centers along with their staff could be utilized to augment the vaccination drive. Drive-through vaccination centers may be considered to improve access and visibility.

Registration on the CoWIN portal

People in the age group of 18-45 years have found it difficult to get registered on the CoWIN portal primarily due to two reasons—the limited availability of slots and the limited number of individuals who use phones. As per the TRAI’s press release for May, 2021, the overall teledensity of India was 85.78%, while the rural teledensity was 59.28%. The teledensity numbers for states like Bihar and UP are in stark contrast to more urbanized states like Delhi. Even those who have phones may not necessarily have smartphones and are often unfamiliar with technology. This puts a considerable proportion of the population directly at a disadvantage. Network issues also act as a deterrent. The problem of slots will be resolved once the availability of vaccine doses improves.

To give equal opportunity to all for registration, the government may consider allowing on-site registration for eligible populations. Stakeholders can also utilize alternate channels like self-help groups, PDS shops, fertilizer dealers, and common service centers apart from routine healthcare frontline workers like ASHAs, ANMs, and AWWS, among others.

India’s food security response to COVID-19

The Public Distribution System (PDS) run by the Government of India provides food security to about 810 million individuals by distributing more than 4 million metric tons of food grains every month. During the COVID-19 pandemic and the resulting lockdown, the government doubled the food grain entitlement of beneficiaries for the first eight months. To distribute the food grains, the central and state governments relied on their established and extensive technology-enabled logistical network with the help of the Food Corporation of India (FCI) and state food corporations. This network allowed officials to keep track of the movement and distribution of food grains in real time. At the last mile, that is, the Fair Price Shops (FPS) or ration shops, the FPS owners with the support from state governments followed safety measures while distributing food grains to the beneficiaries. This enormous task was possible due to a combination of initiatives undertaken by the central and state governments, such as digitization of the supply chain and beneficiary database, improvement in storage and transportation infrastructure, and improved monitoring aided by technology.

How did COVID-19 affect the incomes of migrants and low-wage workers in India?

According to India’s most recent census conducted in 2011, the country is home to 456 million migrants who represent 38% of the total population. The first lockdown to curb the spread of COVID-19 left these migrants stranded in their host communities and desperate to return to their familial homes.[1] Stories of households that lost livelihoods and families that walked hundreds of kilometers to reach their villages dominated the news. Even after reaching their native homes, the dearth of employment opportunities in villages offered migrants little solace.

MSC (MicroSave Consulting) undertook a mixed-method study[2] to understand the impact of COVID-19 on the earnings of low-income households and evaluate the government’s COVID-19 relief package, Pradhan Mantri Garib Kalyan Yojana (PMGKY). We conducted the first phase of the study during the three-month complete lockdown in May, 2020 and the second phase after the relaxation in lockdown norms in September, 2020. The studies covered 5,081 households across 18 states and union territories in each phase, with 4,082 households common to both rounds.

The effect of the pandemic on income and employment

In the first phase of MSC’s study, households across all occupations reported a drop in household income. Chart 1 illustrates the impact of the lockdown on the income of those surveyed. The impact was much more pronounced in May as compared to September, 2020. In May, 82% of households reported a reduction in income.

In September, 2020, MSC asked respondents whether they noticed any change in household income compared to the months of complete lockdown. Their responses are illustrated in Chart 2. 16% reported an increase in income while 10% said they regained employment once the lockdown restrictions were eased. This indicates that conditions had begun to return to normal.

When asked about their status of employment, the responses corroborated the findings mentioned above. In Round 1, 13% of households reported losing their jobs during the lockdown. This percentage fell to 3% in Round 2 and 10% of the households stated that they regained employment.

Due to loss of income, most families had to find other ways to sustain themselves. Many households reported that government benefits helped them stay afloat.

Households reported that circumstances forced them to take loans either from friends and family or from informal money lenders. They repurposed existing loans from self-help groups, banks, and microfinance institutions to manage expenses. People also reported positive experiences with their landlords or money lenders who agreed to delay or forgo rents and interest during the lockdown.

The pandemic had manifold impacts on migrants who depend on daily wages to survive and support their families. These migrants typically work as wage laborers in various industries. The shutdown of these industries due to the lockdown left migrants without any income, which made it difficult for them to pay for food or rent. The Government of India (GoI) did not issue any guidelines to mandate these industries to provide laborers with accommodation or food.

To mitigate the adverse impact on income, GoI launched various government-to-citizen (G2C) payments and in-kind assistance under the umbrella of the Pradhan Mantri Garib Kalyan Yojana (PMGKY). The Finance Minister also announced that the government would allocate USD 468 million (INR 35 billion) to feed stranded migrants. The government further arranged Shramik trains to take migrants back to their native homes. Additionally, in June, 2020, the GoI launched the Garib Kalyan Rojgar Abhiyan across 116 districts in the states of Bihar, Uttar Pradesh, Madhya Pradesh, Rajasthan, Jharkhand, and Odisha to boost employment and livelihood opportunities. This program, with a total expenditure of about USD 5.6 billion (INR 392.93 billion), generated 507,868,671 person-days of employment between March and July, 2020. MSC assessed the impact of all programs under PMGKY and made recommendations on the way forward for each program to shoulder future crises in a series of publications.[3]

The next blog in this series examines the impact of increased allocation to the MNREGA[4] program as part of the GoI’s efforts to support returning migrants by commissioning public works in their villages.

Beyond the barriers of affordability: An analysis of India’s cooking fuel support program under the COVID-19 assistance package

As part of the relief measures announced by the Indian government to combat the COVID-19 crisis, 80 million poor women received cash transfers to purchase liquefied petroleum gas (LPG). This paper analyzes the program’s performance and the experience of beneficiaries while purchasing LPG refills from the money disbursed during and after the lockdown.

Did MGNREGA mitigate the loss in income and unemployment caused by the COVID-19 pandemic?

The COVID-19 pandemic and the resulting lockdown in India had a dire effect on employment and income across the country. The Government of India attempted to mitigate the impact by announcing various relief measures under the Pradhan Mantri Garib Kalyan Yojana (PMGKY). Under PMGKY, the GoI worked to provide relief to a large number of unemployed persons, including migrant workers, who had returned to their villages. These relief measures were channeled through the government’s flagship cash-for-work program, Mahatama Gandhi National Rural Employment Act (MGNREGA). It increased the daily wage rate of workers from USD 2.44 (INR 182) to USD 2.70 (INR 202) and subsequently increased its budget allocation.

MSC (MicroSave Consulting) undertook a mixed-method study [2] the impact of COVID-19 on the income of low-income households and assessed the government’s COVID-19 relief package, Pradhan Mantri Garib Kalyan Yojana (PMGKY). The study covered the effect of the steps taken under MGNREGA. The first and second phases of the study were conducted respectively in May, 2020 during the three-month complete lockdown and in September, 2020, after the lockdown norms were eased. The study covered 1,165 MGNREGA beneficiaries in Round 1 and 1,186 beneficiaries in Round 2 across 18 states and union territories.

The government tried to absorb as many people under MGNREGA as possible. However, the significantly high demand for work made the task challenging. In just 50 days, between 1st April and 20th May, 2020, 3.5 million workers sought employment under the program. In comparison, in the year 2019-20, 1.5 million workers had applied for MGNREGA jobs. Addressing the higher demand, the government provided more work as illustrated in Chart 1, which compares the number of person-days[3] generated under MGNREGA in 2019 against those in 2020. Except in the month of April, 2020 when the lockdown restricted work, person-days generated in 2020 were much higher than in 2019. As per official data, only 15% of households enrolled under the program reported receiving work in round 1 of our study, which increased to 53% in round 2.

One of the biggest achievements of the scheme was that it provided work to women in rural areas. While the Indian government mandates a minimum of one-third participation by women in MGNREGA, 53% of the persons employed under MGNREGA were female in the fiscal year 2020-21.[4]  Additionally, MGNREGA is implemented by local panchayats[5], which must have 50% women representatives. Reservation for women in the program and in its implementing body have led to a massive increase in participation by women.

The government also attempted to make timely payments to the workers. In May, 2020, 40% of respondents reported that they had not received their pending MGNREGA wages so far. In September, 2020, the government ensured that it cleared all dues as 83% respondents reported that they had received all payments pending for the work done in preceding months. According to official data, 87% of the total payments in 2020-21 have been paid.

The government attempted to absorb more people under MGNREGA. However, due to the excessive demand for jobs created by the pandemic, the program could not accommodate everyone. One respondent said, “If 20 people are required for a specific job, more than 200 are available to do the work.” As a result, 31% of respondents who were enrolled in MGNREGA did not receive work after applying.

As more labour was available during the lockdowns, project completion was much quicker under the program. This reduced the number of days required to complete a project—thus reducing the number of days a person was employed and earned wages.  Only 7 million of the 75 million employed under MGNREGA received work for full 100 days in the year 2020-21.

What could have been better?

MGNREGA shouldered some of the adverse effects of the pandemic, providing much-needed employment and income to returning migrants and other vulnerable segments. Its efficacy as a tool to address future economic shocks are likely to improve if some of its design and implementation issues are solved.

  • The scheme is unable to entirely absorb the high demand for jobs created during economic crises. As a result, job seekers do not receive the guaranteed 100 days of work. The government should cater to any surge in employment demand by creating new avenues of employment and expanding areas of permissible work. MGNREGA workers could be employed gainfully in short-staffed positions. For example, they could be instrumental in guarding national parks and forests.
  • Although the GoI raised the daily MGNREGA rate, it should increase further to match the minimum wage rate in each state. The wages paid under the program continue to be lower than the minimum wage rate in 17 states in the country. To do this, the government should increase the budget allocation for the program. The country appears to be moving in the right direction as the allocation for this scheme increased in the 2021-22 Union Budget by 19% compared to the 2020-21 allocation.
  • While irregularities and delays in MGNREGA payment have reduced, these issues can be minimized further through investments in human resources and technology and adoption of automated processes. The Mobile Monitoring System (MMS) should be used across all village councils to update the database on demand submission, work allocation, attendance, and measurement of work. Through these steps, the GoI can curb administrative delays at every stage to provide beneficiaries their dues on time.
  • By design, MGNREGA is only made available to those living in rural areas, which leaves out vulnerable populations in cities. The pandemic dealt a devastating blow to the urban poor. Yet, they were unable to seek employment relief from the government.[6] The pandemic has categorically highlighted the need for an employment guarantee scheme for the urban poor. The government could employ, for instance, urban workers in municipalities to ensure cleanliness and sanitation, and in the process solve the dual problems of unemployment and poor sanitation.
  • Relief provided through MGNREGA or direct income support provides some immediate assistance. Yet it often fails to sufficiently protect the vulnerable from the next shock. As India experiences a second wave of COVID-19 and experts predict a third wave, the government should be prepared with a plan that absorbs the unemployed population. It must also create policies and guidelines for industries to ensure the safety of its workers through the provision of shelter and food so that a huge segment of the population is not forced to return to their villages again.

Lessons for other countries from India’s experience with MGNREGA

Many countries and humanitarian organizations launched cash-for-work programs as a COVID-19 relief measure. India’s 16 years of experience with MGNREGA as an employment support program and now as a COVID-19 relief measure may help budding cash-for-work programs across the globe.

  • Maximum coverage: Cash-for-work programs should ensure coverage of all vulnerable segments, especially during times of crises. The Indian government worked to cater to the rising unemployment and income loss in the country through MGNREGA. However, it could not provide relief to the urban poor as they are not covered under MGNREGA. Similarly in Kenya, the national cash-for-work program, Kazi Mtani continues to target rural youth exclusively. Due to suboptimal coverage, the urban poor are unable to access such relief measures.
  • Gender-inclusive design Economic crises often have a disproportionate impact on women. In our study, households where the primary wage earner is female were reportedly less likely to get reemployed once lockdown restrictions were eased.[7] Governments must ensure that its programs can provide adequate work and income to women. The Indian government mandates a minimum one-third participation of women in MGNREGA. Countries should provide reservation to women in programs and implementing bodies of those programs. They should also ensure the nature of work provided is favorable for them.[8] The World Food Program ensures 60% of those assisted are women in its cash for work program in Tajikistan.
  • Inclusive program design: India employs a localized system where unemployed persons can approach their panchayat to volunteer for MGNREGA work. This allows unrestricted access to every household. Contrastingly, the Pre-employment Card Program announced in Indonesia as a COVID-19 relief measure required individuals to register online. This feature faced criticism as people with low socioeconomic status and low educational levels are less likely to have internet access.[9] Informal workers tend to fall under these demographics, making them less likely to have access to the internet and thus, the program. While digitizing programs, governments should create offline measures too to ensure the most vulnerable are not left out.
  • Transparent system with a strong GRM (Grievance Resolution Mechanism): Communication and lack of awareness remain real problems across the globe. Governments should ensure workers have full knowledge of the amount and timing of payment of wages. They should be able to ask for timely resolution in case of irregular, insufficient, or delayed payments. MGNREGA has an online GRM portal, a publicly available complaint register, and a portal to see the status of complaints. However, the awareness and use of such mechanisms is low among beneficiaries.

Developing nations can use these lessons to better serve their unemployed and low-skilled workers.
